	Banner Space, Sidewalk Chalk and CUB Porch Tabling Reservations Description Fill Out CUB Digital Billboard Description Fill Out This form is a submission form used to enter an advertisement to be displayed on the digital billboards around campus. If you are unsure how to fill this form out please visit this website for instructions on submitting your digital billboard request. Digital Billboard Instructions Request for Cultural Credit Event Description Fill Out For The College of Arts & Sciences and/or The School of Business Cultural Attendance Requirement Event Registration Description Fill Out Register your event at Stetson by filling out an Event Registration form. In order to be approved, this form must be submitted at least two weeks prior to the start date of your event. Before filling out this registration, please make sure your room and venue are booked by visiting the Stetson Reservations Page. 	Scholarship Eligibility Description Fill Out Please complete this form to help the Financial Aid office determine whether you are eligible for a scholarship opportunity. New Officer Training Packet Description Fill Out This form must be completed by officers of new organizations prior to approval or by new officers during officer transition. Fraternity and Sorority Roster Update Form Description Fill Out Use this form to notify Fraternity and Sorority Involvement when a member joins your organization, when a member drops from your organization, and when a member changes their status in your organization. Fill out a separate Roster Update Form for every member you need to update. Organization Student Leadership Assignment Description Fill Out This form must be completed by at least one member of each organization (separate form for each organization) in order to maintain recognition with the Office of Student Involvement. Market Day

	Merchant Application Form for Market Day Spring 2013 Description Fill Out The purpose of Market Day is to allow outside vendors as well as other Stetson student organizations, offices and departments an opportunity to display their goods and services to the entire Stetson Community. Each vendor will be provided (1) table and (2) chairs upon request. Please fill out this form to apply for participation in Market Day.
